Post-Discharge Case Management

Peer-guided case management for the fragile first weeks and months after discharge — a dedicated Recovery Life navigator who coordinates appointments, connects clients to community resources, and keeps the aftercare plan alive, without the clinical hand-off gap that lets people fall through the cracks.

  • A single point of contact who tracks the whole plan — coaching, coping tools, family, and providers
  • Peer-led, not clinical: real lived experience navigating logistics, not paperwork or diagnoses
  • Direct coordination with IOPs, sober living, employers, and community resources during the highest-risk transition window
  • Currently offered through our Toolkit + Kickoff program, with expanded ongoing case management available by application as we grow our team
